{"id":20032,"date":"2021-11-28T08:53:47","date_gmt":"2021-11-28T08:53:47","guid":{"rendered":"https:\/\/www.optometrists.org\/?page_id=20032"},"modified":"2021-11-30T14:07:03","modified_gmt":"2021-11-30T14:07:03","slug":"ocular-surface-disease-or-dry-eyes","status":"publish","type":"page","link":"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-conditions\/dry-eye\/ocular-surface-disease-or-dry-eyes\/","title":{"rendered":"Ocular Surface Disease or Dry Eyes?"},"content":{"rendered":"<h2><b>Scientific studies show that as many as 78% of all adults have some form of ocular surface disease (OSD).<\/b><\/h2>\n<h2><b>What is OSD?<\/b><\/h2>\n<p>Ocular surface disease (OSD) and <a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-conditions\/dry-eye\/what-is-dry-eye-syndrome\/\">dry eye syndrome<\/a> are often mistakenly considered synonymous.<\/p>\n<p><b>OSD occurs when there is damage to the front surface of the eyes, the <a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-health\/how-does-the-eye-work\/ocular-anatomy-the-front-of-the-eye\/\">cornea<\/a>.<\/b><\/p>\n<p>However, OSD is a much broader term, which includes dry eye syndrome, but also refers to a number of other disorders that affect the surface of the eye, and can cause significant issues with your vision and quality of life.<\/p>\n<h2><b>Is OSD serious?<\/b><\/h2>\n<p><b>Yes.<\/b><\/p>\n<p><b>OSD is a major eye health issue that can lead to permanent loss of vision, even blindness.<\/b><\/p>\n<p>OSD occurs when the eye damage occurs and if not treated early can lead to life-long consequences for both vision and eye comfort.<\/p>\n<p>Healthy tear production is the key to keeping your eyes well hydrated, as well as preventing damage to your eyes and keeping them free of dirt and other foreign bodies.<\/p>\n<h2><b>What Causes OSD?<\/b><\/h2>\n<p><b>The most common causes of OSD are malfunctioning of the eye\u2019s water and oil glands and inflammation of the eyelids, known as <a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-conditions\/dry-eye\/eyelid-conditions\/do-i-have-blepharitis\/\">blepharitis<\/a>.<\/b><\/p>\n<p>If any ocular gland is not functioning correctly, it affects tear quality or quantity, leading to dry eye syndrome or other forms of ocular surface diseases.<\/p>\n<p>Specific glands are responsible for your tear quality. The lacrimal gland produces water, and the meibomian glands located inside the eyelids, produce the essential oily layer of the tears. Both glands play an essential role in the production and maintenance of healthy tears.<\/p>\n<p>While dry eye syndrome is among the most common types of OSD, there are several others:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-health\/eyes-and-allergies\/\">Eye allergies<\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-conditions\/dry-eye\/eyelid-conditions\/blepharitis\/\">Blepharitis<\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Decreased tear quality and\/or production with age<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Chemical or thermal burns<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Side effects of medical treatments or prescription medications<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-conditions\/eye-conditions\/corneal-diseases\/keratitis\/\">Keratitis<\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Sjorgen\u2019s disease and other immunological conditions<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-conditions\/dry-eye\/eyelid-conditions\/meibomian-gland-dysfunction\/\">Meibomian gland dysfunction<\/a><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><a href=\"https:\/\/www.optometrists.org\/general-practice-optometry\/guide-to-eye-conditions\/dry-eye\/conjunctivitis-and-eye-infections\/\">Conjunctivitis<\/a><\/li>\n<\/ul>\n<h2><b>Do I have OSD?<\/b><\/h2>\n<p><b>If regular eye drops have not improved your dry eyes, then you most likely have OSD.<\/b><\/p>\n<p>Signs and symptoms of ocular surface disease may vary slightly, depending on the underlying issues.<\/p>\n<p>The majority of ocular surface diseases will have many of the same symptoms, often worsening at night or in windy or dry conditions.<\/p>\n<p>The signs and symptoms of OSD include:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Dry, gritty feeling in the eyes<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Constant feeling like something is in your eye<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Light sensitivity<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Itchy eyes<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Blurry vision<\/li>\n<\/ul>\n<p>In many cases of OSD, as in dry eye syndrome, these symptoms are chronic, and the usual eye drops purchased at a pharmacy will not have had a long lasting benefit.<\/p>\n<p>However in cases where symptoms are the result of injury or irritants in the environment, they will dissipate as soon as the injury is healed or the irritant is questions.<\/p>\n<p>These questions may include:<\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\">When did you first start experiencing dry eye symptoms?<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">How often do you experience symptoms, and are there specific situations or places when\/where they worsen?<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Have you been receiving treatment, or been prescribed medication, for any other\u00a0 medical conditions?<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Have you already tried any treatments for OSD, and did you feel they worked well for you?<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\">Is there a family history of OSD or other conditions that affect the eyes?<\/li>\n<\/ul>\n<p>Once you\u2019ve answered these and any other questions your eye doctor has, they will have a better idea of what might be causing your symptoms. They will then perform in-office testing.<\/p>\n<p>Depending upon the type of OSD your eye doctor suspects, specific dry eye tests might include:<\/p>\n<p><b>Tear break up time (TBUT)\u00a0<\/b><\/p>\n<p>By observing the tears in this way, your eye doctor can evaluate the rate of evaporation of your tears.<\/p>\n<p><b>Corneal examination<\/b><\/p>\n<p>This exam will help the eye doctor diagnose any corneal damage that may have resulted from OSD.<\/p>\n<p><b>Corneal damage can lead to life-long vision problems including ocular discomfort and loss of vision or even blindness.<\/b><\/p>\n<p><b>Meibomian gland evaluation<\/b><\/p>\n<p>Your eye doctor will evaluate your meibomian glands, which are tiny glands lining the edges of your eyelids. These glands produce oil to prevent tears from evaporating too quickly.<\/p>\n<p><b>Phenol red thread test<\/b><\/p>\n<p>This test allows your eye doctor to check if the water layer of your tears has broken down by measuring the volume of your tears.<\/p>\n<p>If the water layer has broken down, tears may not spreading over the eye, giving you a constant feeling of dryness in your eyes.<\/p>\n<p><b>Schirmer\u2019s test<\/b><\/p>\n<p>This is probably the most common test for dry eye syndrome and other types of OSD.<\/p>\n<p>Your eye doctor will place a\u00a0 small strip of medical paper inside your lower eyelid and ask you to close your eyes.
